In this case study, two clinical leaders from St. Joseph Healthcare in Maine, a 112-bed acute care community hospital, will explain how they use predictive analytics to improve care and lower cost. Powered by real-time clinical and utilization data from Maine’s statewide HIE, St. Joseph is able to identify patients at risk for serious illness, high cost, and repeat services. With this information, the provider engages patients early to improve outcomes and prevent serious events.
The results speak for themselves: ED visits are now below the state average and readmissions are below the national average.
