The Community Care Partnership of Maine Accountable Care Organization (CCPM) was cited recently as an innovative model of care by the Rural Health Information Hub, a national clearinghouse of the Federal Office of Rural Health Policy.
CCPM began in 2015 as a partnership between St. Joseph Hospital and Penobscot Community Health Care (PCHC), a Bangor-based Federally Qualified Health Center. Since then CCPM has grown to encompass 11 mission-based hospitals and other health care organizations across Maine. CCPM members strive collaboratively to transform the delivery of healthcare through meaningful sharing and accountability for the health of their patients. CCPM seeks to achieve this goal by improving access to care, improving clinical outcomes, reducing costs, improving patient experience of care and improving employee satisfaction.
